I.DIRECTIONS 

A.   Complete the SponsorshipApplication.  
(INCOMPLETE APPLICATIONS WILL NOT BE CONSIDERED.)

B.    Attach a copy of highschool transcript or certificate of high school credits.

C.    Attach a copy of your ACT test results.

D.   Submit two (2) letters of recommendation with the completed application, one from current high school personnel and one from a community organization.

E.    Submit your typed essay regarding your interest in the oil & gas industry.

F.    Sign the last page of this application.

G.   Submit the completed application to your guidance counselor/senior sponsor no later than April 15, 2009. 

II.   METHOD OF SELECTION 

1. Judging will be performed by a group of oil & gas professionals, and recipients will be notified as soon as possible after the judging has been completed.

2. The recipient will be selected based on the number of accumulated points as designated in each of the categories on the attached forms.

3. In the event of a tie, the two candidates will undergo an interview with personnel from AXIS Onshore, LP. 

III. ELIGIBILITY 

1. The applicants must complete the Louisiana or Mississippi minimum requirements for high school graduation by the end of the academic year.

2. The applicant must have a least a 2.5 grade point average and an ACT composite score of at least 19.

3. The applicant must be interested in pursuing a career in the oil & gas industry.

4. The applicant must be active in both school and community activities
